INTRODUCTION

Facial aging rarely happens in only one layer. As time passes, collagen quality changes, supporting tissues gradually become less firm, the midface may lose some of its youthful projection, and the skin surface can become rougher or less luminous. Fotona4D® was developed as a multi-dimensional approach rather than a single-depth tightening treatment. It combines two complementary wavelengths, Er:YAG and Nd:YAG, through four treatment dimensions that work from the inside of the mouth to deeper facial tissues and finally to the skin surface. This allows the physician to address different contributors to an aged or tired appearance within one coherent protocol.

WHAT’S THE EXPERT SAY?

“For facial rejuvenation, I do not look at tightening alone. Some faces need more internal support, some need deeper collagen stimulation, and others need surface refinement. Fotona4D® lets me balance these dimensions according to the patient’s anatomy and aging pattern. My goal is not to make every face look the same, but to help it look firmer, fresher and naturally more youthful while preserving the patient’s individual character.”

WHAT IS IT?

Fotona4D® is Fotona’s non-surgical facial rejuvenation protocol combining SmoothLiftin™, FRAC3®, PIANO® and SupErficial™. SmoothLiftin™ begins inside the mouth using Er:YAG SMOOTH® mode. FRAC3® uses Nd:YAG energy to create a three-dimensional fractional effect in the skin. PIANO® delivers controlled deep thermal stimulation using an ultra-long Nd:YAG pulse. SupErficial™ completes the treatment with a light Er:YAG surface peel. The four dimensions are designed to complement one another so that tissue firmness, collagen quality, midface support and superficial texture can be addressed within one treatment concept.

Personalization matters because the same laser platform can create different biological effects when pulse duration, energy, spot size, treatment density and number of passes change. Two patients may describe a similar concern but have very different skin thickness, anatomy, pigmentation tendency, previous procedures or tolerance for downtime. A physician therefore needs to select the treatment endpoint rather than simply copying a standard setting from one patient to another.

HOW DOES IT WORK?

SmoothLiftin™ — Intraoral Rejuvenation

Treatment begins inside the mouth, where controlled Er:YAG SMOOTH® energy gently heats the oral mucosa and underlying connective tissue. This can stimulate collagen contraction and remodeling around the nasolabial, perioral and midface regions. Because the direction of treatment comes from inside the mouth, it provides a type of support that cannot be duplicated by an external-only facial laser. The purpose is subtle firmness and natural-looking fullness rather than artificial volume.

FRAC3® — Three-Dimensional Fractional Stimulation

FRAC3® uses Nd:YAG energy to create small, targeted treatment zones within the skin while leaving surrounding tissue relatively unaffected.
This three-dimensional fractional effect supports collagen remodeling and may also help improve the appearance of
pores, redness and minor textural irregularities, depending on treatment parameters.

PIANO® — Deep Thermal Tightening

PIANO® uses an ultra-long Nd:YAG pulse to gradually create homogeneous heating in deeper tissue. Rather than concentrating energy at the surface,
this mode is designed to support collagen contraction and longer-term remodeling at depth.
It is particularly useful when the treatment goal includes overall firmness and support of the lower face or neck.

SupErficial™ — Surface Refinement

The final dimension uses Er:YAG for controlled superficial resurfacing. It may be performed as a very light peel or adjusted according
to the patient’s skin condition and available downtime. This step focuses on roughness, superficial texture and visible radiance,
completing the protocol by addressing a layer that deeper tightening modes do not directly treat.

WHAT RESULTS CAN I EXPECT?

WHAT RESULTS
CAN I EXPECT?

The aim is improvement rather than a standardized transformation. Some changes may be appreciated relatively early, while collagen remodeling or tissue adaptation usually develops more gradually. The final result depends on baseline anatomy, age, skin or tissue quality, treatment parameters and individual biological response.

WHO IS IT FOR?

An ideal candidate is someone whose concern matches the mechanism of the treatment and who has realistic expectations about a non-surgical laser procedure.
A medical consultation is important when there is significant laxity, active disease, pregnancy, recent procedures, medications affecting healing or other clinical factors.

• Early to moderate facial laxity
• Flattening or loss of support around the midface
• Fine lines, enlarged-looking pores or uneven texture
• Patients wanting comprehensive rejuvenation without surgery
• Patients who prefer natural-looking improvement without injectable volumizers

Exact preparation and aftercare vary with the selected parameters. Patients should disclose recent procedures, medications and relevant medical history before treatment. After laser procedures, avoiding unnecessary irritation and following physician-directed aftercare are important parts of recovery.

FAQ

How is Fotona4D® different from a standard tightening laser?

Fotona4D® combines four complementary steps and two wavelengths, including an intraoral dimension. A single-mode tightening treatment generally focuses on fewer tissue levels and does not provide the same inside-to-outside treatment concept.

No. Fotona4D® is a non-invasive laser treatment. It can support firmness and rejuvenation in suitable patients, but it does not remove significant excess skin or mechanically reposition tissue in the way surgery can.

Downtime is usually limited, but temporary redness, warmth, sensitivity or mild swelling can occur. A stronger SupErficial™ step may produce more visible dryness or peeling.

Some patients notice freshness or tightness relatively early, while collagen remodeling develops gradually over the following weeks and months.

There is no universal number. Age, degree of laxity, skin quality, previous treatments and desired improvement all influence the recommended plan.

Yes, when medically appropriate. If a patient also has significant volume loss, pigment problems or more advanced structural laxity, other treatments may be incorporated into a broader personalized plan.
